Regular gnats are nasty little buggers. This one has some particularly wicked personality flaws.
First and foremost, it hits hard and fast... and then it uses Triple Attack. For a safe win, I recommend 2 balanced parties.
Here's the setup I would want for both parties:
PLD/WAR (or PLD/NIN if both have it available)
BLM/WHM (curing/stun/sleep/late nuking)
RDM/WHM (refresh/enfeebling and backup curing... this is an endurance fight)
RDm or WHM (main healing... SCH?)
DD x2
A PLD/WAR with tacos has an incredible amount of defense to bring to bear against the murderous damage this thing dishes out, as well as Provoke and Warcry for keeping hate. PLD/NIN has the advantage of both Utsusemi versions and enough defense to survive when shadows are down.
Because of Emetic Discharge, I recommend using minimal enfeebles. I wouldn't want more than 2 or 3, at a time. Paralyze, for sure. Other good possibilities are Slow, Gravity (for evasion penalty), Blind (though I doubt the effect would be noticeable), Silence (to stop those wicked Drains, if you can stick it).
Of the 4 DDs, one should be THF, to increase the drop rate. I would want the other 3 to be 2-handed DDs (or MNK) and /thf. The DDs can't afford to pull hate and the PLDs can use the extra hate so that the mages are free to keep them alive.
If you get into trouble, don't give up. Keep it asleep while everyone recovers, then go back to work.
We killed this with a 6-person party. It was a grueling, chaotic battle that I never want to repeat. *I* don't want to go back there with less than 12 people.
